If you are already a donor and have donated €250, you could make your donation grow by up to 44.9% by completing a tax relief form and returning it to us at the address below.

The tax relief form is called a CHY3 Enduring Certificate and it lasts 5 years. This means that you only need to fill out the form once every five years which reduces our admin costs allowing more funding to reach our vital services for people with physical disabilities.

What information do I need?

All you need is your own name, address and PPS Number and the name of our charity which is Irish Wheelchair Association.

How do I know if I qualify?

You are able to help us with this extra support if you:

  • Have donated a total of €250 or more to Irish Wheelchair Association in one year (Jan to Dec) in the last four years.
  • Are a taxpayer in Ireland

Does signing a CHY3 form mean that I have to commit to donating to IWA?

No, the five year form is not a commitment on your part to donate anything – it just means we can claim tax relief if you do decide to donate a total of €250 or more in any of the five years covered by the form.

What if I’m a pensioner?

If you have a pension or any other investments that means you pay either income or capital gains tax. Therefore, yes your donation is still eligible for tax relief.

I don’t pay PAYE but my spouse does – am I still eligible?

Yes, as a married couple either of you can sign the form. Just remember to ensure that you put down the details for the person who does pay PAYE when filling out the form.

How much extra will go to Irish Wheelchair Association?

Your donation will be worth up to an additional 44.9%. This means that if you’ve donated €250, we can claim up to an extra €112.33 from revenue. This means your original generous donation would now be worth €362.33 at no extra cost to you!

Should I tick the “associated with approved body” box?

No, you only need to tick that if you are an Irish Wheelchair Association employee or board member during the year your donation was made.

My tax details are private. Will this information be confidential?

Yes, your details remain completely confidential to Irish Wheelchair Association and Revenue. The information on this form will not be used for anything besides the tax claim.

The Difference Your Signature Makes - Mary’s Story

Mary is in her 50s and lives in Dublin. A graduate of Trinity College Dublin, she paints, writes and has been a disability campaigner and advocate all her life. She manages her own Personal Assistant Service, coordinating her own care and hiring her own assistant. Over the years, Mary’s medication has become more complex. She worried that she might be unable to remain living independently if she couldn’t get support – and she hates the thought of losing her autonomy.

Without a PA service, or the backing of the Clinical Team, I would be in an institution. There’s no doubt about it.” she says. “For me, an institution would break my spirit.”

IWA’s National Clinical Support Team began working with Mary in 2012, offering extra support with looking after her medication. Working with this team means Mary isn’t depending on one PA who might get sick or need to take time off work. She feels in control and has a greater sense of independence.

Even just €112.33 – the minimum additional value we can claim from one valid tax form – could help provide vital training to a Personal Assistant and support someone with a disability to live independently.
